    Too many rpms

    Seen it quite a few times where a wheel is turned way too fast.

    Saw a chunk about 1/2 that size bounce off tool box and drive into an a-holes forearm who would not listen when told the grinder he mounted the wheel onto turns 10x faster than what the wheel shows as max. rpm.
